Gosh darn it, I fell behind again! Here is #creatober entry 14.
Box of Polymorph Self
This large box is big enough to hold a full sized human, and somewhat resembles the dimensions of a coffin. It is inscribed with strange arcane runes, and elaborate heavy ironwork clasps. Should a creature voluntarily enter the box and shut the lid, the following round they will emerge as if affected by the polymorph spell. However, what they have become must be rolled randomly on the following table:
